We will work on a high-level picture of the current quality of care in the United States and how six aims for improvement from the Institute of Medicine have created a sense of direction for improvement efforts all over the world, and an introduction to the science of improvement, with a theory of how to change systems.

Six domains of healthcare quality
Although a number of analytic frameworks for quality assessment have guided measure development initiatives, the following one produced by the Institute of Medicine (IOM) is helpful to include six aims for the healthcare system.
- Review the Six Domains of Healthcare Quality.
Avoid harm to patients from the care that is intended to help them.
Provide services based on scientific knowledge to all who could benefit. Refrain from providing services to those not likely to benefit (avoid underuse and misuse, respectively).
Provide care that is respectful of and responsive to individual patient preferences, needs, and values and ensure that patient values guide all clinical decisions.
Reduce waits and sometimes harmful delays for both those who receive and those who give care.
Avoid waste, including waste of equipment, supplies, ideas, and energy.
Provide care that does not vary in quality because of personal characteristics such as gender, ethnicity, geographic location, and socioeconomic status.
